With clinical inertia, sliding scale insulin therapy in hospitals remains pervasive and episodes of hyperglycemia and hypoglycemia still occur. There is a significant opportunity to improve glycemic management and adopt recommended guidelines by conducting a gap analysis as the first step in the process; however, hospital staff and administration need to understand that significant performance improvement does not occur without a change in the way that people do their work. A gap analysis is a comprehensive and strategic approach to assessing an organization’s current state of glycemic management knowledge and practice, identifying gaps between the current state and an ideal future state, and developing a strategic plan to close those gaps. This plan allows people to do the work that matters, resulting in fast and recognizable improvements in patient outcomes, often with significant reductions in cost of care.
